Terms and Limitations of the plan:
- Cannot substitute member for another person
- Your effective date is the day of sign up. The renewal date is 365 days from the sign up.
- All dental treatment must be completed before the end of the coverage date to qualify for the discount.
- This is a dental membership discount plan, and cannot be combined with dental insurance.
- Payment is due at the time of service for any treatment not covered at 100%.
- Rates are subject to change annually.
- Membership dues are non-refundable.
- Membership is only valid at Kirol Family Dentistry.
- Cancellation fee applied if less than 24 hour notice.
- Discount does not apply to products.
- Membership dues to be paid in full upon purchase of plan.
Membership Dues:
- Adult Member (age 14 and over on date of purchase)- $400 annually
- Adult Perio Member- $600 annually
- Child Member (age 13 and under on date of purchase)- $365 annually
